We take a streamlined, focused approach to support individuals with diverse disability-related needs, especially during the intake process. Here’s how we do it:

Assessment:

When someone joins our program, we start by gathering essential information through an intake form. This helps us shape a personalized plan for them.


Review:

We carefully check the individual’s records to see if they have any health conditions that might affect our support. We also look for past treatments and assessments to better understand their situation.


Collaboration:

Our review process is a team effort, involving the individual, their support network, and family. Together, we pinpoint the person’s strengths, skills, and goals. We focus on positive aspects and use clear language to identify challenges.


Goal Setting:

Once we have a clear picture, we work on establishing specific goals. We create an action plan that outlines responsibilities and any special services or equipment needed to reach these goals.


Referrals:

In the unusual event where NL Services is unable to provide a specific requested service, NL Services will help guide the individual towards the correct services they seek.


Communication:

We maintain ongoing communication with the individual and their support network to ensure our services meet their changing needs.


Empowerment:

We believe in giving choices and involving the individual in planning their support. We respect their rights and aim to help them live their best life by understanding and meeting their unique needs and desires.
